Instructions to fill the Form

1. Instructions about cancellation:

If the cancellation application is submitted on or before 05 - 10 - 2009, Rs. 500/- will be refunded per person.
Cancellations made after 05 - 10 - 2009 will not be refunded. However, if one has to cancel, it is necessary to inform and return the ID cards.
Cancellation forms can be obtained from Yogi Sabhagruh (on the day of the Param Satsang) and at Bombay Market (except Sundays).

2. One form is to be filled per family. Friends and relatives accompanying you must fill separate Forms.
3. Underlined fields in the form are compulsory.
